Frontline workers experience the operating system rather than only its official description. They see missing inputs, workarounds, contradictory instructions, unsafe shortcuts, and recurring exceptions. The key failure is not simply that leaders do not listen; it is the absence of a closure loop that captures the observation, tests it, assigns ownership, acts, and reports disposition back.
